The recall was prompted by the death of a 29-year-old woman who slid down the Banzai Falls slide headfirst and broke her neck when the slide partially deflated and bottomed out. Her injuries were so severe that she died the next day. Two other people had similar accidents resulting in very serious neck injuries.

Upon investigation, the CPSC determined the slides are defective and can deflate suddenly, causing the user to crash to the ground. Further, they found the slide is unstable and can topple over in both still and windy conditions. Finally, they determined that it contains inadequate instructions and warnings.

How can I tell if my pool slide is included in the recall?

The recall is just for the Banzai in-ground pool slide—an inflatable slide designed for use with an in-ground pool. The slide has a blue base, yellow sliding mat, and an arch going over the top. The words ‘Banzai Splash’ are printed on both sides of the slide in a wave-shaped blue, orange, and white logo. A hose connects at the top of the slide in order to spray water down the slide.

The slides were manufactured in China by Manley Toys, Ltd, and sold for about $250 at Toys R Us and Wal-Mart stores between January 2005 and June 2009. They contain barcode number 2675315734 and model number 15734, which are printed on the original packaging but not on the slide itself.

My slide works fine. Why should I return it?

According to the CPSC, just because you are not currently having problems with your Banzai Falls in-ground pool slide does not mean that it is safe. It can collapse at any time and cause serious injury to you or someone you love. You are urged to stop using the slide immediately.
